Some payroll and leave requests require approval from a member of the Administrative Committee (AC). Following our recent divisional reshuffling, we wanted to ensure your requests are routed to the correct leader for timely approval. Use the document below to identify which AC member oversees your specific department.
Direct Deposit is the most secure and efficient way to receive your pay. This form allows the College to electronically transfer your earnings directly into your chosen checkings or savings account(s) on payday. Use this form if you are a current employee who needs to change banks or update account numbers. You can choose to deposit your entire paycheck into one account or split your pay across multiple accounts.
FMLA (Family Medical Leave Act) is a federal protection that provides eligible employees with up to 12 weeks of job-protected, unpaid leave per year for qualified medical and family reasons.
Use this form if you need time off for the birth or adoption of a child, to care for a family member with a serious health condition, or for your own serious health condition that prevents you from performing your job. It ensures that your health insurance stays active and your job (or an equivalent of one) is waiting for you when you return. Read more about FMLA here.
The Holiday Payout request ensures that employees who work non-traditional schedules or essential roles receive their full holiday benefit. Supervisors use this form when an employee works the holiday and needs to be compensated for that time elsewhere. If you are a supervisor who has employees within this scenario and do not have access to the form, please contact the Payroll Systems Administrator in Huma Resources for assistance.

PLA (Parental Leave Act) is a Berea benefit designed to support employees following the birth, adoption, or foster placement of a child. Use this form if you are the primary or secondary caregiver looking to utilize your paid leave benefit. This leave runs concurrently with FMLA but provides actual paid time to ensure you can focus on bonding with your new family member without financial strain. Read more about PLA here.
This form is used by eligible new employees to formally request and receive their approved, one-time gross relocation payment. New employees whose signed appointment letters includes a negotiated relocation allowance or are eligible subject to a qualified moving distance. This allowance request must be made within one year of the hire date, and employees who resign within two years of their hire date must repay the gross allowance amount (repayment schedule: 100% for the first year, 50% for the second year).
Stipends represent non-standard, supplemental pay for specific periods, such as an extra duty assignment, a special-project, or a pre-approved bonus. They are not part of a regular base salary. Since stipends often cover unusual durations, the Stipend Calendars below are provided to identify the specific pay dates when these unique lump-sum payments will be issued. A stipend payment is initiated by a direct supervisor or divisional vice president. They must send in a request through Human Resources.

Berea College recognizes that unexpected life events can sometimes exhaust an employee’s accrued leave. Our Vacation Donation program allows our community to support one another by voluntarily donating vacation hours for colleagues who may be in need. Please read the policy in full here.

If you are facing a serious medical emergency or a qualifying life event and have exhausted all of your own paid leave (including sick and vacation time), you may request a donation of hours from the campus community.

If you wish to “pay it forward” by gifting a portion of your accrued vacation time to the general donation pool, use the form below to specify the number of hours you wish to contribute. Donors must maintain a minimum balance of their own vacation leave to ensure they have enough time for their own rest and recovery.
